The Department of Housing and Residence Life works in coordination with the Disability Services Office to provide accommodations to any resident with differing abilities or special needs. Admitted and currently enrolled students who wish to recieve accomodations for a disability should submit documentation from their health care providers using the Office of Disability Services Documentation Form provided below.
Personal Attendant Notice
If you will require the services of a personal attendant during your stay with us, please follow the steps outlined below for Requesting Services.
You will also be required to complete a Background Check Certification Form on the attendant who will be living with the resident. This form is required for all persons who will live or work in our residence halls to provide health-related services to a resident. It is the responsibility of the resident (or his or her legal guardian) to have a background check completed. For assistance with the form, please contact the Assignments Office. The form is available for download at the bottom of this page.
In order to ensure that all requests are properly handled in accordance with UNC Charlotte's policies and procedures, as well as State and Federal laws, we kindly ask that students contact the Office of Disability Services to submit their requests.
